
e are pleased to announce that SculptraŽ Aesthetic has been approved for use as a cosmetic filler for facial rejuvenation. This product has already been available for a number of years to restore volume loss in patients being treated for AIDS, also known as HIV-related lipoatrophy. Over the last 10 years, there has been a revolution in our understanding of the changes that occur in our faces as we age.
We understand that a youthful face is a fuller and rounder face, and so we now approach facial rejuvenation surgery with this in mind. No longer is the emphasis only on pulling the skin tighter and tighter as with traditional facelift surgery. Now the emphasis is on restoring volume as well as lifting sagging skin. In some cases, restoring volume by itself will create a fuller lifted appearance. In other cases adding volume will be done in conjunction with traditional face-lifting techniques.
Look at pictures of yourself going back 10, 15, or 20 years, and you will immediately notice how the face has lost volume. This may be particularly noticeable in the cheeks, lips, and around the eyes and temple areas.
SculptraŽ Aesthetic is uniquely suited for more generalized restoration of facial volume, unlike products like products like Juvéderm and Collagen, which are more suited for filling lines and wrinkles and enhancing lips. SculptraŽ Aesthetic is placed more deeply than these materials and is therefore more suited for a more diffuse volume enhancement. SculptraŽ Aesthetic is also unique amongst fillers in that it stimulates your own body to make more collagen resulting in a longer lasting improvement.
Dr. Jeffrey Hartog, Orlando cosmetic surgeon, has more than 15 years of experience restoring facial volume, and the most common materials used for this purpose are SculptraŽ Aesthetic, Radiesse (made from hydroxyapatite similar to the crystals in bone) and fat injection.
Usually, Dr. Hartog combines SculptraŽ Aesthetic with Radiesse to provide an ideal facial volume enhancement. The benefit of using these two materials in combination is that the Radiesse will provide an immediate volume enhancement so that you can see the improvement immediately, while SculptraŽ Aesthetic will restore volume in a more gradual but longer lasting fashion.
Now using the Vectra advanced 3-Dimensional computer imaging, we can actually demonstrate the effect of adding volume to make you look younger and more attractive. After capturing a 3-D image on the computer, we can add volume to the desired areas to show you the effect we can achieve, and also to help plan the amount of volume required.
An example of this treatment is shown. Initial Vectra 3-D imaging images are shown demonstrating the severe hollowness in the cheek, temple areas and around the eyes. The before picture shows the pronounced hollowness in the cheeks, temples and around the eyes. Using the computer sculpting tools volume is added to these areas, and the anticipated results can be visualized once volume has been fully restored.

Actual before and two month after the first treatment pictures are shown and the effects of combined SculptraŽ Aesthetic and Radiesse injections can be seen demonstrating the predictive value of the 3-D imaging. Another 2 or 3 treatments spaced approximately 3 months apart are planned to further enhance and maintain the result. After about a year, only occasional touch-up treatments should be required.
