Low- Level Laser Assisted Liposculpture

The Bougainvillea Clinique is very excited to announce the introduction of Laser Assisted Liposculpture, the first plastic surgery clinic in Central Florida to do so. This remarkable new liposuction technique was discovered quite by accident by Dr. Rodrigo Neira from Cali, Columbia. Having observed a presentation of Dr. Neira's technique, Dr. Hartog at The Bougainvillea Clinique immediately recognized the potential benefits for his patients, These benefits include less bleeding and bruising, less postoperative discomfort, and it would appear, smoother enhanced skin retraction. Dr. Hartog now routinely uses this procedure in addition to the usual tumescent and superwet liposuction techniques and has been extremely impressed with the improved smoothness of his results, enhanced skin retraction, and obviously less bleeding, bruising and discomfort. The technique uses a low power laser that is already FDA approved for treating pain in the United States, and has been used to help with wound healing as well. Dr. Neira in Columbia discovered this technique after he had purchased one of these lasers to help treat his liposuction patients and decrease their pain and swelling after the procedure. Quite by accident he noticed that after treating a liposuction patient with this low level laser, liquid fat leaked from their liposuction incisions. He subsequently demonstrated that by treating a given area with this laser for 12 minutes prior to performing liposuction, the fat was almost miraculously released from the cells, resulting in easy, almost bloodless removal of this liquefied fat during liposuction. More than two years later, and having now performed over 1000 procedures with this technique, Dr. Neira has demonstrated this technique to be safe, with many advantages over previous liposuction techniques. His results demonstrate remarkable skin retraction, even with large volume liposuction. Using electron microscope studies he has also demonstrated how this low level laser treatment releases fat from the fat cells without causing any damage to the cells themselves or to other structures. Unlike ultrasonic liposuction, no heat is generated by this procedure, so that there is no risk of burns or other damage to tissues. Amazingly, the fat cells themselves are not even destroyed when the fat is released, and after they are removed by the liposuction, they can actually be used for fat grafting and will survive and re-accumulate fat. At the Bougainvillea Clinique we are also using this technique to help speed up the recovery of all surgical patients, by treating the areas with this same laser after surgery, as it was originally designed to do. Additionally, we are using this technique together with endermologie to help improve the appearance of cellulite and enhance skin tone.

Breast Augmentation and Lift
- using an adjustable implant for better healing and improved scars.

Combining breast augmentation with a breast lift is often the best procedure for those who have deflated and sagging breasts. If the nipple is at or above the breast fold, then usually an augmentation alone will fill and lift the breast, however if the nipple is below the fold, then frequently a breast lift is required as well to provide an ideal shape. If a small augmentation is needed, then a silicon gel implant is often an ideal solution to provide the most natural appearance. On the other hand, many women seeking breast augmentation want more than a small augmentation, and this can sometimes lead to problems when the two procedures are combined.

A mastopexy involves an incision around the areola, and a vertical incision if the lower breast skin needs tightening as well. Placing a medium or large implant at the same time as the mastopexy procedure can result in excessive tension on the wounds as they heal, resulting in delayed healing at times, poor scars, and even loss of nipple skin occasionally.

To decrease the initial tension on the incisions I now use the Mentor Spectrum Adjustable implant with all combined mastopexy augmentation using saline implants. If the patient chooses to have silicon gel implants, and agrees to participate in the appropriate gel implant study protocols, then I will use gel implants, but only up to 250cc. The Spectrum implant has a valve, which can be connected to the implant through a thin tube, and the valve can be left outside the skin for 7-10 days, allowing the implant to be filled and adjusted after the procedure.

The procedure involves placing the implant in the usual fashion, usually submuscular, and completing the mastopexy. At the end of the procedure the saline is removed so that only 150cc is left in the implant. At the one-week postoperative visit, the implants are inflated to the desired final fill volume, and then the fill tubes are removed with a gentle tug. Occasionally, if there is a question about symmetry, or with very large implants, we may take a few days to inflate the implants.

I have found this approach to have many advantages, the most significant of which is a tension free wound, which results in better healing, and much better scars. Even just one week of tension free healing, allowing swelling to subside before inflating the implants, seems to have made a great improvement in the quality of the scars. This has been my experience with circumareolar and vertical scar mastopexy procedures. In addition, I have found this technique valuable in obtaining better symmetry as well, as the implants are adjusted after much of the surgical swelling has subsided.

Breast Augmentation: The TUBA Procedure

At the Bougainvillea Clinique we educate our patients on the pros and cons of different procedures, so each individual can decide which option best fits his or her needs. Over the last few years there has been increasing interest in the TUBA procedure among breast augmentation patients - this is the procedure that allows the implant to be placed through an incision in the umbilicus(belly button). "TUBA" stands for Trans Umbilical Breast Augmentation.

Until recently, patients choosing the TUBA procedure were only offered subglandular breast augmentation (under the breast tissue). At the Bougainvillea Clinique we offer both submuscular and subglandular TUBA procedures depending on the individual's desires and their breast and body shape. Remember the TUBA is just another place to put the incision and way to access the breast area to place the implant. The decision on where to place the incision should never compromise the ability to get the best possible shape of the breast and best possible placement of the implant. In general, the inframammary (under the breast) and periareolar (lower part of the areola) incisions still allow the surgeon the best access to the breast area. We will determine if the breast shape needs to be changed or improved significantly, and recommend the best course of action. It may also not be the best choice if the individual desires large implants. Remember larger implants require more extensive work to create the pocket for the implant, so correct positioning becomes much more important.

The next best incision (besides the fold and nipple incisions) for good access is the axillary (armpit) incision, followed by the TUBA procedure. Both of these procedures require the use and availability of an endoscope (surgical telescope) so the surgeon can visually check the pocket he is creating. Some surgeons do the TUBA and the axillary approaches without the availability of an endoscope, which obviously limits the ability to see exactly what has been done within the pocket. Other surgeons claim that the TUBA procedure is scarless - this is obviously untrue. The incision and scar are simply now in the belly button area. For some this is an advantage, for others it may not be their preferred choice. We think the TUBA procedure is best suited for individuals who have good shape and symmetry to start with, and who are requesting a small to moderate enlargement. The TUBA procedure may be a bad choice for someone with significant uneven size and shape of the breasts, poor shape to start with, or someone who wants a large augmentation. Remember if there are problems with the augmentation, there is a strong likelihood that an additional inframammary or periareolar incision will be required to fix the problem. This is a potential disadvantage of the TUBA and to a lesser extent the Transaxillary procedure.

As with all the choices regarding breast augmentation, we always recommend you understand the pros and cons of every option. Your surgeon should be able to provide these options, even though as a well-trained surgeon he may make strong recommendations as to what he thinks is the best option for you. Avoid gimmicks, be careful of choosing a procedure just because it is new. This same philosophy applies when considering round or anatomical implants.

The vast majority of breast augmentation scars heal well with very few complaints from patients in our experience. Similarly every area has the potential for a noticeable scar - yes even the axillary and TUBA procedures can have a noticeable scar occasionally.

  • Will likely require a second incision in the unlikely situation that significant bleeding occurs during surgery, or if capsular contracture needs to be surgically treated later.
  • More difficult than the areola or fold incisions to fix uneven breast position, more difficult than the areola or fold incision to accurately create a pocket for a large implant.

Temporal Brow and Midface Lifts
Look in the mirror; if you're 35 to 45 years old, chances are you will notice some drooping of your brow and cheeks, and an increase in the skin fold on each side of your nose.

Lipostructure
Recently featured in USA Today, restoring the structure of youth with fat grafting is now a reality. If you look at a young face, it's round and full, not angular and hollow. As we age, our faces become smaller; almost like balloons deflating, and as they deflate, the skin wrinkles and sags.

To see how this has occurred in your face, look at photographs from 5, 10, 15 and 20 years ago. Using your own fat many of these signs of aging can be reversed. The technique of lipostructure gently harvests your own fat in a similar way to regular liposuction. The fat can then be reinjected where needed through small specially designed needles, into the lips, nasolabial folds, chin, and around the eye and temple areas.

Other defects in other parts of the body, including liposuction problems, can also be improved with this technique. The procedure can be done in one sitting, or the fat can be frozen and reinjected in smaller amounts every few weeks. This is a very safe procedure, and can often be done under local anesthetic alone.
