The following are questions
frequently asked by our patients.

If I am having breast augmentation,
is it better to go over the muscle or under the muscle?
There are advantages and disadvantages of both approaches, depending
on your build; at the Bougainvillea Clinique we provide both options.
Most of the data indicates that the risk of hardening around the implant,
(capsular contracture) is significantly lower under the muscle, and
there is much less chance of having noticeable rippling in the upper
part of the breast where it would be noticeable in a swimsuit or low
cut dress. We also perform the 'no-touch' technique which seems to significantly
cut the risk of hardening. On the other hand, women with larger, lower
set breasts, may get a better appearance over the muscle, unless they
are prepared to have a breast lift as well.

If I am having breast augmentation,
which incision is best?
There is no best incision. The incision position is your choice! We
explain the advantages and disadvantages of the 4 most common approaches;
under the breast, around the areola of the nipple, under the arm, and
the TUBA (belly button) incision with endoscopic (surgical telescope)
control. All of the incisions work well, but we will go over your options
and decide what works the best for you depending on your preference.

Are saline implants safe and
how long do they last?
Any individual undergoing breast augmentation must be aware of the two
important issues with regard to possible long term problems with implants.
The first is that an implant will most likely deflate sooner or later;
fortunately the implant manufacturers give lifetime warranties on the
implants, and so this is more of a nuisance than a real problem. The
average lifespan of an implant is about 15 years. The other concern
is the risk of hardening, and for this reason we recommend under the
muscle, if your breast shape will accommodate this. Other problems such
as infection, can happen but are extremely rare.

Can I breastfeed after having
breast implants?
Though occasional problems with breast feeding have been reported, this
is extremely rare. And by far the majority of women who have implants
have no problem breast feeding normally.

How do I know whether I need
a breast lift or breast enlargement?
The most important consideration in this regard, is the position of
the nipple. If the nipple is at or above the breast fold, then a breast
augmentation alone will give you adequate lift. If the nipple is below
the fold, then most likely a lift, with or without an augmentation,
will be needed to give the best breast shape.

Liposuction and Body Contouring
What is the difference between
tumescent and ultrasonic liposuction?
Liposuction techniques have evolved over the last 15 years, allowing
superior results. Newer techniques result in better skin contraction
and smoothness, with less blood loss and quicker recovery. Also much
larger volumes of fat can be removed with safety, compared to early
techniques. The tumescent and superwet techniques, performed at the
Bougainvillea Clinique, involve injection of a saltwater solution which
has local anaesthetic solution and small amounts of adrenalin in the
solution. Ultrasonic liposuction is the latest form of liposuction,
which also uses the tumescent or superwet technique, but in addition
uses an ultrasonic probe to break up the fat before it is removed. For
small liposuction procedures, ultrasonic liposuction is probably unnecessary,
ultrasonic liposuction is superior in large volume liposuction, previously
difficult areas like the back and enlarged breasts in men, or when additional
skin contraction is needed.

Does liposuction help loose
skin and cellulite?
Loose skin will contract to a variable degree with modern techniques
of liposuction. Moderate cellulite can sometimes be improved. The limiting
factor with liposuction however has always been the ability of the skin
to contract. At the Bougainvillea Clinique, we have demonstrated that
superior results can be achieved with the addition of Endermologie after
liposuction. The benefits are enhanced skin contraction and smoothness,
and significant improvement in the appearance of cellulite.

Is liposuction permanent,
and will the fat come back?
Once fat is removed it will not come back, but liposuction will not
prevent you from gaining weight if you do not watch your diet. Remember,
if you do allow yourself to put on weight, the fat that is left behind
will of course expand, though probably to a lesser degree than if you
had not had liposuction. At the Bougainvillea Clinique we help with
weight loss, anti-aging and nutritional programs as well as Endermologie
and Pilates body conditioning to help you stay looking good after liposuction.

People say the fat comes back
in other places, is this true?
This is not true, however if you allow yourself to gain weight, you
will most likely gain weight disproportionately in untreated areas.
This may give the impression that the fat is coming back in other areas.
After puberty, most of us gain weight by storing more fat in the fat
cells we have, not by getting more fat cells. So the untreated areas
will be able to store more fat than the areas that have been treated
with liposuction. There is also some evidence that after liposuction,
an individual's metabolic 'setpoint' may be lower, making it easier
to keep weight off. Once again, at the Bougainvillea Clinique we provide
numerous treatments and programs to help you enhance and maintain your
result.

Facial Rejunvenation and Skin Care
How long does a facelift
last?
Most experts agree that the average facelift lasts 8-10 years, but I
would rather that patients thought of it that the procedure will make
them look 8-10 years younger, and that the clock starts running again
after the procedure. At the Bougainvillea Clinique we strongly encourage
our patients to take advantage of programs that will significantly extend
the time that their facial rejuvenation procedures will last. A balanced
medical skin aging process to a significant degree. Other anti-aging
and lifestyle interventions available at the Bougainvillea Clinique
will go a long way to keep you looking younger.
I do not want a facelift yet, but is there anything I
can do to take care of some of these lines and the little bit of sagging
skin I see in the mirror? There are many options available to make a
significant difference without a complete facelift. These include a
skin health program, laser or chemical skin resurfacing, and minimally
invasive endoscopic techniques. At the Bougainvillea Clinique, we will
explain the options available to you. If a facelift really is the best
option we will of course recommend it too.

Why do I have brown patches
and spots on my skin?
Can I do something about it? The most common cause of uneven skin discoloration
is sundamage. The first step is to protect your skin from the sun, and
then to do something about reversing the sun induced damage and aging
changes. Treating the deep layers of the skin with anti-aging products
such as Retin-A, and adding blending products to help smooth uneven
discoloration, followed by a procedure like the Obagi Blue TCA Peel
or even laser resurfacing, can dramatically improve the health and appearance
of sun damaged skin.

How do I know whether I need
a tummy tuck or liposuction for my stubborn midsection?
Whether or not a tummy tuck or liposuction is best for you depends on
how much extra skin you have around your abdomen, and what you are trying
to achieve. If your only wish is to take off inches, then ultrasonic
liposuction can go a long way to helping you achieve this without the
larger tummy tuck procedures. If on the other hand you have a significant
amount of excess skin and you want the tightest possible tummy, then
a tummy tuck may be your best option. At the Bougainvillea Clinique
we will help you make the decision regarding the best procedure for
you.

How do I know if I need liposuction
or Endermologie?
The way your figure looks depends on the amount and condition of fat
and skin. Liposuction removes the fat, but the ultimate outcome depends
on the ability of your skin to shrink. Endermologie helps improve the
condition and tone of your skin, enhancing any liposuction result. Endermologie
will not remove significant bulges of fat, but it will help smooth,
tone and tighten the skin and improve the appearance of cellulite. That
is why at the Bougainvillea Clinique we recommend Endermologie for all
our liposuction patients who want to look and feel their best. If fat
is the problem, then liposuction is appropriate, if poor skin tone is
the problem then Endermologie will help tighten the skin. For most people,
fat and skin are an issue, so both liposuction and Endermologie are
recommended. If the skin is really loose and in poor condition, then
only skin excision procedures such as tummy tucks and body lifts will
help.
